The Australian Transport Safety Bureau (ATSB) has said shipping operators must regularly check and replace their container lashing equipment after it concluded an investigation into an incident onboard the Pacific Adventurer.

In March 2009, the Hong Kong-registered container vessel lost 31 containers overboard during bad weather off Cape Moreton in Queensland.

The unsecured containers punctured holes in two of the ship’s fuel oil bunker tanks, leading to a 270t-oil spill.

The ATSB said the Adventurer‘s loose and fixed container lashing equipment was in poor condition and the inspection and replacement regime of the equipment had not been implemented.
